Urusan Konsular - (Passport,Visa,Perkahwinan,Pengesahan Dokuman,Pendaftaran Rakyat Malaysia Luar Negara)

Consular-related Services


All Malaysian nationals who are in need of the consular – related services from the Embassy of Malaysia are kindly requested to bring with them copies of the following documents, in addition of the originals:


1- PASSPORT:


(A) New Passport Application.

1- Copy of the current passport.
2- Copy of the I/C.
3- Copy of the Birth Certificate.
4- Educational attestation (Tasdiq), for students only.
5- Two personal photos (blue background and passport size).
6- Form (obtained at the office of the Embassy).

(B) Passport Renewal Application.


1- Copy of the current passport.
2- Copy of the I/C.
3- Form (obtained at the office of the Embassy).

(C) Malaysian Travel Document (Emergency Certificate).


1- Copy of the current passport.
2- Copy of the I/C.
3- Form (obtained at the office of the Embassy).
4- Police Report (in case the original passport is lost).
5- Air ticket (in case of emergency travel).

Notice: New passport application should be done before six month (or earlier) from the date on which the current passport expires.







2- MALAYSIAN BIRTH CERTIFICATE (BORANG W)


1.Two copies of the father's passport.
2.Two copies of the mother's passport.
3.Two copies of the father's I/C.
4.Two copies of the mother's passport.
5.Two copies of the father's birth certificate.
6.Two copies of the mother's birth certificate.
7.Two copies of the marriage certificate.
8.Two copies of the baby's Egyptian birth certificate.
9.Two personal photos of the baby.

Notice: Registration of birth must be done within the first year.


3- DRIVING LICENSE

1- Original Passport

-Pasport
-Visa
-Perkahwinan
-Pengesahan Dokumen
-Pendaftaran Rakyat Malaysia Luar Negara

Semua mahasiswa /wi diminta membawa dokumen yang dilampirkan diatas sekiranya mempunyai urusan yang berkaitan dengan kedutaan untuk program pada esok hari.(MESRA'10)
